Herbal Compound Eases Kidney Scarring by Restoring Fat Burning in Rats
A compound from a traditional Chinese kidney formula restored fat-burning activity in kidney cells and eased scarring in rats, a new study found.

- What they found: In rats with blocked ureters, a compound from notoginseng reduced kidney scarring by helping kidney cells burn fat normally again.
- Who was studied: Rats with surgically blocked ureters and lab-grown kidney cells exposed to a scarring-triggering protein.
- What it doesn't show: Whether it works the same way in people.
A compound derived from notoginseng eased kidney scarring and restored normal fat metabolism in kidney cells in rats with blocked ureters, a lab model of chronic kidney disease, according to a study published October 28, 2026 in Phytotherapy Research.
Researchers studying chronic kidney disease, in which scar tissue progressively replaces healthy kidney tissue, tested notoginsenoside R2, an active compound isolated from the traditional Chinese Shen-Yan-Fang-Shuai formula. The study, using rats with surgically blocked ureters (a standard model for triggering kidney scarring) and kidney cells exposed to a scarring-triggering protein called TGF-β1, found that the compound eased kidney dysfunction and scarring in the rats and reduced fat buildup, oxidative stress, inflammation and collagen deposits in both the animals and the cells. Sample size was not reported.
The authors report that notoginsenoside R2 works by binding a protein called caspase-2 and helping it stabilize a second protein, ALDH3A1, instead of triggering caspase-2's usual role in cell death signaling. Stabilizing ALDH3A1 restored the structural connection between mitochondria and a nearby cell compartment, and that repair allowed kidney cells to burn fat normally again using proteins called CPT1A, ACADL and PPARα. In kidney scarring, damaged cells often lose the ability to burn fat efficiently, and fat accumulates inside cells, feeding the stress and inflammation that drive scar formation. Restoring that fat-burning step, as the authors describe it, is what interrupted the scarring process in this study.
NGR2 upregulates CASP2, enhances the CASP2-aldehyde dehydrogenase 3 family member A1 (ALDH3A1) interaction, and promotes ALDH3A1 deubiquitination
This is animal and lab-dish evidence using a surgical model of kidney blockage, not naturally occurring human kidney disease, so it does not show whether the compound works the same way, or is safe and effective, in people with chronic kidney disease. Sample sizes, dosing details and the exact degree of improvement were not reported in the abstract.
Dr. Axe’s Take
I love seeing a traditional Chinese formula get this kind of cellular detail. Shen-Yan-Fang-Shuai already shows kidney-protective effects in clinical use, and this work shows how its key compound works in rats: it restores how kidney cells burn fat, which interrupted the scarring process. For kidney health this week, I'd focus on keeping blood sugar and blood pressure in check, eating whole foods over processed ones, and getting regular kidney function labs if you have risk factors. If you already have kidney disease, keep taking your prescribed medications as directed and check with your nephrologist before adding any herbs.
