Metabolism-Matched Feeding Preserves More Muscle After Stroke In Trial

A small hospital trial found stroke patients fed to their measured calorie needs kept more facial muscle thickness than those fed a standard weight-based formula.

A quasi-randomized trial published October 15, 2026 in the British Journal of Nutrition found that feeding acute stroke patients according to their individually measured calorie burn preserved more facial muscle thickness by day 14 than standard weight-based feeding.

Researchers at a single hospital assigned 74 acute ischemic stroke patients needing tube feeding by odd or even hospital admission number; 6 were excluded before allocation and 68 were split into a group fed to 120% of measured resting energy expenditure (via indirect calorimetry, a breathing test measuring calories burned at rest) or a control group fed 25 kcal per kilogram of body weight daily. The complete-case analysis included 54 patients, 24 in the measured-metabolism group and 30 in the control group. After adjusting for age, sex, baseline stroke severity score, total calorie intake and day 0 inflammation levels, left temporal muscle thickness (a facial muscle used as a marker of overall muscle wasting) was 0.56 millimeters thicker in the measured-metabolism group by day 14. Right temporal muscle thickness and calf circumference did not differ significantly after adjustment; unadjusted calf circumference was actually lower in the measured-metabolism group. Prealbumin, a blood protein reflecting recent nutrition status, trended numerically higher in that group but missed significance. Albumin, transferrin, inflammation markers and complication rates showed no meaningful differences.

Stroke pushes the body into a hypermetabolic, muscle-wasting state that burns through protein and energy reserves faster than normal. Matching calorie delivery to a patient's measured metabolic rate, rather than a generic weight-based formula, was linked to better preservation of one muscle marker in this trial.

This single-center trial used hospital admission number rather than true randomization, with only 54 patients analyzed after exclusions. Only one of several markers reached significance after adjustment, and the authors call the biomarker findings exploratory. Temporal muscle thickness stands in for whole-body muscle, not functional recovery.

Dr. Axe’s Take

Generic weight-based calorie formulas can miss how differently people burn energy after a stroke. This trial's result backs that, though the win is modest, one muscle marker out of several, and the trial is small, so I would not call this settled. The logic matches what I recommend for anyone recovering from serious illness: adequate protein and individualized calorie targets rather than a one-size-fits-all chart. If a family member is hospitalized after a stroke, ask the care team whether metabolic testing is available and whether the feeding plan is protein-forward.
