The best keto foods are non-starchy vegetables, extra-virgin olive oil and other healthy fats, avocado, wild fish with fins and scales, pasture-raised eggs, grass-fed meat, nuts and seeds, and goat, sheep or coconut dairy, eaten so your net carbs stay around 20 to 50 grams a day.1

Keto, short for ketogenic, is a way of eating that keeps carbohydrates low enough that your body switches from burning sugar to burning fat and the ketones your liver makes from it.1 The name goes back to 1921, when Dr. Russell Wilder of the Mayo Clinic first used the diet to treat epilepsy and coined the term "ketogenic diet."6

What are net carbs, and how many can you eat on keto?

Quick answer

Net carbs are total carbohydrate minus fiber, and most keto plans keep them to about 20 to 50 grams a day.16 Staying near 20 grams gets most people into ketosis within a few days, and some active people stay in ketosis closer to 50.1

Fiber is the part of a plant's carbohydrate your body can't break down into sugar, so it doesn't raise your blood sugar, and that's why it comes off the total.1 More fiber helps you. In a review of 42 trials in 1,789 adults with prediabetes or diabetes, eating more fiber lowered HbA1c by 2 mmol/mol (about 0.2 percentage points) and fasting blood sugar by about 10 mg/dL, compared with lower-fiber diets.7

That's why a plate piled with greens, broccoli and zucchini fits keto so easily. Half a cup of boiled broccoli carries 3.0 grams of net carbs, and a medium baked sweet potato carries 19.8.2

A typical keto split is about 75% of calories from fat, 20% from protein and 5% from carbohydrate; the exact split varies by person, and carbohydrate can run up to 10%.1 Protein stays moderate because the body can turn very high amounts of it into glucose, which blunts ketosis.1 See keto macros for how to set your own numbers. Here's what happens in your body once net carbs stay that low:

Your body can no longer rely on sugar as its main fuel.16

Your body burns through its stored sugar, called glycogen, over a day or two.1

3Insulin falls

With little sugar coming in, insulin falls and your body pulls more of its energy from stored fat.1

4Your kidneys release sodium and water

Insulin helps the kidneys hold on to sodium, so as it falls, more sodium and water leave your body, which is why salt and minerals matter in the first weeks.81

5Your liver makes ketones

The liver turns fat into ketones, mainly beta-hydroxybutyrate, which your brain, heart and muscles can burn for fuel.1

6Ketosis, then fat adaptation

Most people reach ketosis within 2 to 4 days, and becoming fully fat-adapted takes a few weeks.1

On keto you eat non-starchy vegetables, healthy fats and oils, fish, eggs, poultry and grass-fed meat, full-fat goat, sheep or coconut dairy, nuts and seeds, avocado, olives and small amounts of berries.1 Sugar, grains, starchy vegetables, beans and most fruit come off the plate while you're in ketosis.1

Fish, eggs and meat carry next to no carbohydrate. Beef liver is the exception on this list, at 4.4 grams in 4 ounces raw, so count it on liver days.2 About 3 ounces once a week is fine, unless your bloodwork shows vitamin A running high.

Keep red meat fresh, unprocessed and grass-fed. The WHO's cancer agency classifies processed meat of every kind, processed beef included, as a Group 1 carcinogen, largely on colorectal cancer data, and rates red meat itself Group 2A, probably carcinogenic.9 As I've said on my show, the studies that link red meat to poor health mostly reflect grain-fed and processed meat, not the fresh, grass-fed red meat I recommend.10

Dairy and dairy alternatives

If you eat dairy, choose full-fat, ideally fermented dairy from goats or sheep, such as aged cheeses and plain yogurt; conventional cow's-milk dairy sits on the avoid side of my food lists, and cultured A2 cow's-milk dairy is an option if you tolerate it.11 For cream in coffee or cooking, use canned coconut milk or coconut cream. On my show, I've ranked canned coconut milk as the number one milk for most people.12

Avocado, olives, lemon and lime behave like a fat or a seasoning on keto. Berries are the sweet fruit that fits, in small amounts: half a cup of raspberries or blackberries carries about 3 grams of net carbs, and half a cup of blueberries 8.9.2 See keto fruit for more.

Leafy greens, broccoli, cauliflower, zucchini, asparagus, peppers and mushrooms carry so much fiber and water that their net carbs barely register: 0.4 grams in a cup of raw spinach and 1.1 in half a cup of boiled cauliflower.2 The people who struggle on keto are usually the ones who drop their vegetables.1 They also supply much of the potassium, magnesium, folate and vitamin C on a keto plate.1

Their fiber is a big part of why they belong at the center. In the fiber review of 42 trials, which raised fiber with foods or with supplements such as psyllium and guar gum, more fiber improved blood sugar and also lowered triglycerides and body weight compared with lower-fiber diets, and the benefits weren't limited to one type of fiber.7 Eat them raw in salads and cooked as the main side at every meal.1

Extra-virgin olive oil

Olive oil is my everyday fat, and you can use it far more liberally than tallow or butter.1334 Its fat is mostly monounsaturated, it carries protective polyphenols, and a tablespoon adds 13.5 grams of fat and no carbs.12

It has been tested on heart attacks and strokes themselves. In PREDIMED, a randomized trial of 7,447 adults at high cardiovascular risk, a Mediterranean diet supplemented with extra virgin olive oil lowered a combined measure of heart attack, stroke and cardiovascular death by about 30% over a median of 4.8 years, compared with a reduced-fat control diet.14 That diet wasn't keto, though the oil is the same one you'd pour over a keto plate.

Aim for about 1½ to 4 tablespoons a day, in place of other fats rather than on top of them.4 Cook with it at normal heat, finish dishes with a raw drizzle, and use avocado oil for very high-heat searing.4

Avocado

Half an avocado carries 1.8 grams of net carbs, 14.7 grams of fat and 6.7 grams of fiber, so it brings fat, fiber and potassium to the plate in one food.21

In a review of 9 randomized trials with 503 adults, eating avocado lowered total cholesterol by 5.08 mg/dL, and in people who started with high cholesterol, it lowered LDL by 9.4 mg/dL; LDL didn't change in people whose cholesterol was already normal.15 Its potassium also helps replace what you lose in the first weeks of keto.1 Add half an avocado to eggs, salads or a bowl of greens with lemon and salt.

Wild fish with fins and scales

Wild salmon, sardines, mackerel, herring and anchovies give you protein and omega-3 fats with no carbs: a can of sardines packed in oil, bones included, carries 22.7 grams of protein, and 3 ounces of cooked wild Atlantic salmon carries 21.6.2 For seafood, I hold to one standard: wild, ocean-caught fish with fins and scales.11

Oily fish has been tested as a food. In a randomized trial of 126 Chinese women aged 35 to 70 with high triglycerides, an 80-gram fillet of Norwegian Atlantic salmon or herring at lunch 5 days a week for 8 weeks lowered triglycerides by 25% with salmon and 23% with herring, against 9% in the women who ate a usual mix of meats.16 Salmon and herring also lowered TNF-alpha, a marker of inflammation, from where the women started, and all three oily fish tested raised blood omega-3 levels.16

Pasture-raised eggs

A large egg carries 6.3 grams of protein, 4.8 grams of fat and 0.4 grams of carbohydrate.2

Eggs have been tested inside a low-carb diet. In a randomized trial of 28 overweight and obese men aged 40 to 70 who ate 10% to 15% of their calories from carbohydrate, those who added 3 whole eggs a day for 12 weeks saw HDL, the protective cholesterol, rise from 1.23 to 1.47 mmol/L (about 48 to 57 mg/dL), while HDL didn't change in the men who ate an egg substitute. LDL didn't change in either group, and across the study the number of men with metabolic syndrome fell from 18 to 3.17 Choose pasture-raised eggs, and eat the whole egg, yolk included.1

Nuts and seeds

Macadamias, pecans, walnuts, chia, flax, hemp and pumpkin seeds are the lowest-carb picks: an ounce of pecans carries 1.2 grams of net carbs, an ounce of macadamias 1.5 and a tablespoon of ground flaxseed 0.1.2

In a review of 61 controlled trials of tree nuts in general, with 2,582 adults, each daily serving lowered LDL by 4.8 mg/dL, ApoB by 3.7 mg/dL and triglycerides by 2.2 mg/dL.18 Keep your servings to about a handful a day at most, the amount I recommend, and soak almonds to make them easier to digest.11

What fats should you eat on keto?

Quick answer

Extra-virgin olive oil is the everyday fat on keto, with avocado oil for high-heat cooking, coconut oil and MCT oil for ketones, ghee in moderation, and grass-fed butter and tallow in small amounts.419311 Refined seed oils, trans fats and fried foods stay out.1

Fat is your main fuel on keto, and its quality decides what kind of keto you're doing.1 Pure oils carry no carbohydrate, so choose them for what they do in your body. On my show, I ranked my top 10 cooking fats; here's where the keto fats land and how to use each one:13

Beef tallow is fine in small amounts for many people, but it isn't an ideal fat for heart health or a fatty liver, so for most cooking, let olive oil carry the load, ahead of both tallow and butter.34

MCT oil concentrates the medium-chain fats found in coconut oil, and it's a fast way to raise ketones for energy and appetite control.1 Large amounts at once can loosen your stools, which is why you start small and build up.19 For more, see the best keto fats.

What foods should you avoid on keto?

Quick answer

Added sugar, refined grains, processed meats, refined seed oils, sweetened drinks and packaged "keto" snacks stay out for good, along with pork and shellfish.111 Beans, whole grains, starchy vegetables, most fruit and honey are good foods that wait until your keto phase is over, because one serving can use up most of a day's carbs.2

These come off the plate and stay off:1

  • Sugar: Soda, candy, desserts, syrups and sweetened drinks.
  • Refined grains: White bread, pasta, crackers and boxed cereal.
  • Processed meat, charred meat and pork: Bacon, sausage, deli meats, hot dogs, every cut of pork, and any meat charred or blackened over high heat.
  • Refined fats: Refined seed oils, trans fats and fried foods.
  • Alcohol: Beer, sugary cocktails and spirits. Wine is the one optional exception, covered below.20
  • Packaged "diet" and "keto" products: Low-fat and sweetened diet foods, and ultra-processed keto bars and snacks.

Hitting your macros with processed meats and packaged snacks is what's called dirty keto. It can get you into ketosis while skipping the nutrients that make keto worth doing.1

Shrimp, crab, lobster, oysters, clams, scallops and squid don't meet the fins-and-scales standard I follow, so choose wild fish instead.11 See fish you should never eat for the full skip list.

Healthy carbs to set aside for now

These are good foods. Beans, lentils and properly prepared whole grains are eat-often foods on the biblical diet, which sweetens with honey and fruit.20 They come off the plate during keto only because of their carbs:

Which sweeteners and drinks fit keto?

Monk fruit, stevia and allulose are the sweeteners that fit keto. They rank right after raw honey, my number one sweetener when used sparingly, but a tablespoon of honey carries 17.3 grams of net carbs, so it waits until your keto phase is over.21202 Even then, Scripture's advice on honey holds: "Hast thou found honey? eat so much as is sufficient for thee" (Proverbs 25:16, KJV).20

I put sugar alcohols, erythritol included, in my "Sinister Six," the six things to stop eating first, because they're heavily processed and can cause gas, bloating and headaches. Sorbitol and maltitol sit on my avoid list, and xylitol belongs only in small amounts, such as in gum or mints.11 Diet sodas and other artificially sweetened products stay out too.1 For more, see keto sweeteners.

For drinks, keep to water, sparkling water, unsweetened tea, coffee and bone broth.1 Wine is optional, for people who already drink, in moderation; there's no health reason to start, and many should avoid it entirely.20 See keto drinks for more ideas.

What does a keto diet do for your health?

Quick answer

Keto lowers insulin and switches your fuel to fat, and in trials it has helped with weight, blood sugar and triglycerides.1222324 In a review of 13 randomized trials, people on very-low-carb ketogenic diets lost about 2 pounds more over a year or longer than people on low-fat diets.22

Weight. The review of 13 randomized trials followed 1,415 adults for at least 12 months on ketogenic diets of no more than 50 grams of carbohydrate a day. Next to low-fat diets, they lost 0.91 kg (about 2 pounds) more, with triglycerides about 16 mg/dL lower, diastolic blood pressure 1.43 points lower and HDL about 3.5 mg/dL higher.22 See keto for weight loss for more.

Blood sugar. In a review of randomized trials in adults with type 2 diabetes, low-carbohydrate diets under 130 grams a day, keto included, brought 57% of people (76 of 133) into remission at about 6 months, against 31% (41 of 131) on comparison diets, which were mostly low-fat. Remission meant an HbA1c under 6.5%, with or without medication, and this result pooled 8 trials with 264 people.23 The gap was smaller, and could have been due to chance, when remission also required being off diabetes medication or was measured at 12 months.23

Blood fats. In a review of 62 randomized trials, keto lowered triglycerides by about 20 mg/dL, raised HDL by about 3.6 mg/dL and improved the triglyceride-to-HDL ratio, and it also raised LDL by about 8.5 mg/dL on average.24 The LDL rise tends to be largest in lean people: in a review of 41 randomized trials of low-carb diets under 130 grams a day, with 1,379 people, LDL climbed 41 mg/dL in groups whose average BMI was under 25 and fell 7 mg/dL in groups with a BMI of 35 or more.25 ApoB, a count of the particles that damage arteries, is a sharper heart-risk marker than LDL alone, so if you have cardiovascular risk, recheck your lipids, including ApoB, after 8 to 12 weeks.1 See ApoB vs. LDL for more.

Four markers worth checking on keto

These tests show how your metabolism is responding to keto. No single result tells you whether keto is right for you.

  • HbA1c: Your average blood sugar over about three months, the headline marker for diabetes and prediabetes.
  • Fasting insulin: An early window into insulin resistance, often high long before blood sugar rises.
  • Triglycerides: Blood fats that usually fall on keto, reported on a standard lipid panel.
  • ApoB: The number of cholesterol-carrying particles that can lodge in artery walls.

How long should you stay on keto?

Keto works best as a season of eating. On my show, I put it this way:

After that, add healthy carbohydrates back and bring your fat down to a moderate level.5 For weight loss, I've said keto can work very well over 3 to 6 months.5 My reasoning is that eating any macronutrient in very high amounts for a long time tends to tax an organ system, and for keto, that's the liver and gallbladder.26 Doing keto for 30 or 90 days, on the other hand, can be great.26

Listen to your body along the way. What nutrients run low on keto?

Quick answer

Sodium, potassium, magnesium and fiber run lowest on keto, and a plate built on vegetables, avocado, nuts, seeds and fish, plus salt on your food, covers most of the gap.1 In a review of 10 studies of carb-restricted diets, intakes of thiamine, folate, magnesium, calcium, iron and iodine fell 10% to 70% from where people started.27

The fatigue, headaches and irritability some people feel in the first days of keto mostly come down to electrolytes, lost with water as insulin drops.18 Add sodium, potassium and magnesium from day one: salt your food, sip broth, and eat avocado and leafy greens.1 Typical amounts are about 3 to 5 grams of extra sodium a day, 1,000 to 3,500 milligrams of potassium from food or a supplement, and 300 to 400 milligrams of magnesium.1 A 2025 review of symptoms at the start of keto found a sound physiological case for electrolytes, though clinical studies documenting their effect are still lacking.28 If you take blood pressure medication, read the safety section before adding the extra salt or a potassium supplement.

Constipation is the most commonly reported single side effect: in a review of 36 studies of ketogenic diets, it made up 24% of all the side effects recorded.29 Chia, flax, avocado, plenty of vegetables and water are the fix.1

Who should be careful with keto?

Quick answer

Keto needs medical guidance, or should be skipped, if you have type 1 diabetes or take an SGLT2 inhibitor, insulin, a sulfonylurea or blood pressure medication, if you're pregnant or breastfeeding, or if you have a gallbladder or pancreas condition or certain rare metabolic disorders.301

Take extra care with keto if any of these apply:
  • Type 1 diabetes or an SGLT2 inhibitor: Keto combined with an SGLT2 inhibitor, a class of type 2 diabetes drugs that includes empagliflozin, can trigger diabetic ketoacidosis even when blood sugar looks normal. In two documented cases, one patient developed it after a single dose while eating very low-carb, and another after one week of keto on the drug.3130 The doctors who reported those cases advise avoiding keto while you're on one of these drugs, so talk with the doctor who manages your diabetes before changing your diet.31 With type 1 diabetes, start keto only with that doctor's guidance.30

  • Insulin or sulfonylureas: Blood sugar can drop quickly once carbs come down, so your prescriber needs to adjust your doses as your readings improve.301

  • Blood pressure medication: Keto can lower blood pressure, so doses may need lowering. Check with your prescriber before adding the extra salt keto calls for or a potassium supplement.301

  • Pregnancy and breastfeeding: A strict ketogenic diet isn't recommended without medical guidance, and the research on keto while breastfeeding is still limited.130

  • Gallbladder removed, or gallstones: A sudden jump in fat can be hard to digest without a gallbladder, and with existing gallstones, a high-fat diet makes it more likely that a stone moves into the bile ducts, so raise your fat gradually.130 For anyone without a gallbladder or with gallbladder issues, I recommend bile support, 125 to 500 milligrams of bile salts with fatty meals, and on a high-fat diet such as keto, it can help if fat sits heavily.32331 If you add digestive enzymes, choose plant- or microbe-based ones.

  • Pancreatitis, liver failure and rare metabolic disorders: Skip keto with acute pancreatitis, liver failure, porphyria, or an inherited metabolic disorder, such as a fatty-acid oxidation, carnitine or pyruvate carboxylase disorder.301

  • Advanced kidney disease, familial hypercholesterolemia or a history of disordered eating: Do keto only with medical oversight, and with an eating-disorder history, a gentler approach is wiser.130

  • Red flags: Stop and seek care right away for severe abdominal pain, persistent vomiting, or high ketones with very high blood sugar, or with normal blood sugar if you take an SGLT2 inhibitor.131

What foods can you eat on keto?

Start every meal with non-starchy vegetables, add healthy fat led by extra-virgin olive oil and avocado, and fill in with wild fish, pasture-raised eggs, poultry, grass-fed meat, nuts, seeds and a few berries.1 If you eat dairy, choose full-fat goat or sheep dairy, ideally fermented, or canned coconut milk.1112

How many carbs can you eat on keto?

Most keto plans keep net carbs to about 20 to 50 grams a day, and staying near 20 grams gets most people into ketosis within a few days.1 Some active people stay in ketosis closer to 50.1

What are net carbs?

Net carbs are total carbohydrate minus fiber, the part of a food's carbs your body can't turn into sugar.1 An ounce of pecans has 3.9 grams of carbohydrate, 2.7 of them fiber, so its net carbs come to 1.2.2

What vegetables can you eat on keto?

The non-starchy ones: leafy greens, broccoli, cauliflower, zucchini, asparagus, Brussels sprouts, green beans, peppers, mushrooms, cucumber, celery and cabbage.12 A cup of raw spinach has 0.4 grams of net carbs, and half a cup of boiled broccoli has 3.0.2

Can you eat fruit on keto?

Yes, in small amounts: berries fit, and so do avocado, olives, lemon and lime.1 Half a cup of raspberries has 3.3 grams of net carbs, while a medium banana has 23.9, so most other fruit waits until you add carbs back.2

Is cheese keto?

Yes: an ounce of hard goat cheese has 0.6 grams of net carbs, and an ounce of cheddar has 1.0.2 Choose full-fat cheese from goats or sheep.11

Are beans keto?

Not during a keto phase, because half a cup of boiled black beans has 12.9 grams of net carbs.2 Beans and lentils are healthy everyday foods outside of keto, and they come back when you add healthy carbs to your plate.20

Is honey keto?

No: a tablespoon of honey has 17.3 grams of net carbs, close to a full day's carbs on a strict keto plan.2 Outside of keto, raw honey, used sparingly, is my number one sweetener; on keto, use monk fruit, stevia or allulose.2120

Is erythritol keto?

Erythritol is a sugar alcohol, and I put sugar alcohols, erythritol included, in my Sinister Six, the six things to stop eating first, because they're heavily processed and can cause gas, bloating and headaches.11 Monk fruit, stevia and allulose are the better keto sweeteners.21

What can you drink on keto?

Water, sparkling water, unsweetened tea, coffee and bone broth all fit keto.1 Beer, sugary cocktails and spirits are out, and wine is optional, in moderation, for people who already drink.120

How much MCT oil should you take on keto?

Start with 1 teaspoon a day and build to 1 to 2 tablespoons, stirred into coffee, smoothies or dressings rather than used for cooking.119 Large amounts at once can loosen your stools.19

Is peanut butter keto?

Peanuts aren't on this keto food list, so reach for tree nuts and seeds instead.1 Two tablespoons of smooth peanut butter has 5.5 grams of net carbs, while an ounce of pecans has 1.2 and an ounce of macadamias 1.5.2

Can you eat bacon on keto?

Not on clean keto: bacon is a processed meat, which the WHO's cancer agency classifies as Group 1, and I keep pork and processed meats out of keto entirely.9111
